Constipation and Withholding
This issue which often causes real misery for the whole family. I have worked with many parents who sometimes tell me that it has been going on for 3 or 4 years. What is clear is that there is very little support and information available to parents and GPs are often very uninformed and proper effective action needs to be taken. There is some useful advice given by a charity called ERIC which you might want to take a look at. It is important to distinguish that constipation and withholding are two different issues - often the constipation has triggered the withholding but they should be addressed separately. Withholding is when a child is actively stopping themselves from passing a poo. It has usually been triggered by passing some painful poos (as a result of being constipated) and this then sets off a behavioural pattern that causes them to both consiously and unconsciously stop themselves from pooing. It really needs addressing as fast as possible as it can carry on for a very long time.
When I work with parents I want to understand everything about their child - their character and temperament, likes and dislikes, favourite activities, books and book/tv/film characters as well as diet and eating patterns as all these will form part of the solution. As a parent, I’m sure you’ve figured out by now even if you only have one child, that every child is so different so I to work out a bespoke solution a ‘one size fits all’ approach will not work.
When we I work with clients to get this issue sorted - which can be fast if the problem only recently started or a while longer if it has been going on a very long time - the difference in the child is phenomenal. It is like a flower unfurling, they start to engage more with the world, their confidence takes a huge leap, they sleep better, eat better and play better. They stop feeling shame and embarrassment, they stop lying and covering up accidents. (which is very common with withholding children particularly the older ones) and they feel proud of themselves and their capabilities.
I teach parents how to work with their child to embrace the problem together, for everyone to understand the anatomical and physiological workings of the body, how to have good routines and habits that a child wants to cooperate with. We also look closely at diet, getting the right medication prescribed and hydration.
